Transportation: How to Get to a Provider Appointment or to Get Medical Treatment (MEDICAID)

You can get transportation to and from your medically needed visits. All you have to do is call LogistiCare to set up transportation for your healthcare needs or log onto the LogistiCare website. Please set up transportation to and from your routine appointment at least three days in advance. For more urgent appointments, please call LogistiCare to determine if you can be transported without the 3 day notice. They will let you know after they confirm with the provider if the appointment is urgent. LogistiCare also offers reimbursement for gas if you have your own transportation. Please contact them to see if you qualify and for more information about the program.

• Log onto https://member.logisticare.com to schedule you own routine transportation.

If you have special transportation needs such as a stretcher, door-to-door services, trips more than 50 miles, or out of state trips, please let LogistiCare know as soon as possible. These special requirements require additional approval. Transportation services are only provided to and from your medically necessary provider and dental appointments. If you will need to stop at a pharmacy, please tell LogistiCare when you make your reservation.

Please note: you must provide appropriate child safety seats for your child to use when using LogistiCare transportation. Reservations cannot be made and drivers will not transport without the child safety seats available.

LogistiCare can drop off and pick up from these locations:
• Your home
• A doctor or dental office
• A clinic or hospital
• Other place of medical service, including a pharmacy

If you have an emergency and need transportation, call 911 for an ambulance.

As a value-added benefit, INTotal Health provides FAMIS members transportation to and from medically needed visits. INTotal Health covers up to 10 round trips (20 trips in one direction) per member per year. All you have to do is call LogistiCare to set up transportation for your healthcare needs. Please set up transportation to and from your appointment at least three days in advance. Please note: you must provide appropriate child safety seats for your child to use when using LogistiCare transportation. Reservations cannot be made and drivers will not transport without the child safety seats available.

If you have special transportation needs such as a stretcher van or door-to-door services, trips over 50 miles, or out of state trips please let LogistiCare know as soon as possible. These special requirements require additional approval call Member Services at 1.855.323.5588 and ask for case management. A case manager can help arrange these types of transportation. Transportation services are only provided to and from your medically necessary provider and dental appointments.
